Here are a few suggestions that I have picked up from my personal video experience.
1. I'd suggest starting off with your partner holding the camera. This is known as POV for Point Of View in the porn business. It has a couple of advantages. First, he can actually see what is being filmed and adjust the camera to capture what he (and you) want. Second, watching it will give you his perspective on the acts. Third, it tends to capture somewhat more graphic footage than a distant camera. It's disadvantages are that he actually has to hold the camera and you may not want close ups. I'd suggest after a bit switching to a stationary camera so he can return to a natural rhythm. Put it beside the two of you if you want to see your identities clearly, or behind you to give you some anonymity. The camera behind will capture very graphic insertion if your partner is on top and you wrap your legs around him.
2. I wouldn't worry about being self conscious. Editing software is very common and easy to use these days. If you find that a piece of the footage isn't flattering, just cut it out. In the end, this isn't going to be a Hollywood production anyway.
3. I do not take the extreme position that you shouldn't let yourself be filmed. I try not to be a hypocrite. It doesn't hurt however to minimize the potential embarrassment by keeping your face out of the footage. After all, you and your partner already know who is on the film.
