Ask Dan & Jennifer

I'm 16, male and haven't kissed a girl before. I have a girlfriend now and she knows that (I think I've hinted it clearly without fully saying it) and I think she has before (she has dated and the first time I asked her out she told me she wasn't ready cos she still liked her ex). We've been going out for over a week now and still haven't kissed (is that normal?) mostly because at the beginning I was confused as the "not ready for a new boyfriend" line hadn't been removed yet so I wasn't sure whether I should have broken off so I didn't come off as just friends or go for it and come off as pushy. When it came up one time (she asked me whether it was OK that her best friend was a guy because it used to drive other people she'd been with mad) I saw that the times just me and her went out alone together (twice at that point) were dates. Now we've been on a few dates and we get the college bus together, see each other when we can in college and we've held hands before, hug all the time, people know we're going out together but we still haven't kissed. I feel like I've missed opportunities like one time we were alone(ish) at a park on a hill together just snuggling for warmth and I feel like that was missed but that was our first date (just after going the cinema) one time it was raining and dark so we sat together on a bench alone under her umbrella under a tree and talked and she asked me what I brought her there for and I think maybe that was a chance. Also, she'll snuggle up to me and rest her head on my shoulder and I rest my head back on hers. I'll try to see the signs of her looks up every now and then. I'm taller than her too which is awkward for times were standing she'll hug me and I can rest my chin on her head. I don't want our first kiss to be something crap and boring like at the cinema, at a bus stop, or on the bus etc etc. It's my first kiss and I want to remember it like it was our first kiss and make it as memorable for her as her first as it will be mine. Is that too much too ask?
What should I do? I'm too shy if I'm missing chances! Will she make the first move? Should I ask her can I kiss her? How can I turn a conversation into me kissing her? Where is a good place and time? What's the best way to do a first kiss (90% you 10% her? I think I know how to do the actual kissing bit but tips to make absolute sure would be nice)?
